The revised Uniform Hazardous Waste Manifest is a hazmat shipping form prepared by all generators who transport, or offer for transport, hazardous waste for off-site treatment, recycling, storage, or disposal. When completed, it contains information on the type and quantity of the waste being transported, instructions for handling the waste, and signature lines for all parties involved in the disposal process. The hazardous waste manifest is required by DOT and EPA. The Uniform Hazardous Waste Manifest is available in 6-part snap-out and pin-feed formats. Continuation sheets (Cont.) can be utilized when shipping more than four hazwaste products. Both snap-out and pin-feed formats are provided in sequentially numbered forms. Personalized forms are also available for the Uniform Hazardous Waste Manifest.
